Here’s what you can expect at the first visit:
In the initial intake, you will tell your story and what led up to you or your child’s current symptoms. Your records will be reviewed, additional lab testing will be ordered if necessary, a physical exam is done and an individualized treatment plan is given. This may include: diet changes, lifestyle counseling, stress management techniques and supplements. Pediatric patients will often be given their homeopathic remedy at this visit. Please bring any recent records that you have along with any supplements that you are currently on, if possible.
We will email the necessary paperwork to you once a new patient appointment is scheduled. Please return the completed forms back to us via email or fax at least 48 business hours prior to the appointment.
The doctors and staff at Blossom Wellness Center strive to stay on time, as your time is just as valuable as theirs. We kindly ask for you to arrive 15 minutes prior to the scheduled appointment time.
Office Policy for Insurance Reimbursement
At this time, we will provide you with all the necessary forms to self-submit to your insurance for possible reimbursement. We are looking for options to provide additional insurance billing services in the near future.
